Several Community Behavioral Health Centers across the country are training case managers to become Health Navigators. Health navigation expands the traditional case manager role from helping consumers with serious mental illness establish a life in recovery to a broader role of assisting them to learn to manage chronic physical illness. The mental health field has spent more than 40 years perfecting the skills necessary to assist people in learning to manage a chronic illness—their mental illness. Those same skills—helping consumers accept and overcome the challenges of mental illness, navigate the system, and learn or relearn life skills—are transferrable to chronic physical health conditions such as diabetes, heart disease, and chronic obstructive pulmonary disease. In fact, behavioral health case management may involve the exact skill set needed to assist primary care in moving from addressing issues of chronic illness by treating acute episodes to adopting a preventive, recovery-oriented approach to physical health problems. Workshop Description:

Addictions service providers are called upon more and more to work with clients from diverse backgrounds. In this interactive, skill-building workshop, participants will be introduced to an individualized person-centered approach to engaging clients from diverse backgrounds. Topics covered include: The Cultural Competence Continuum; How to Develop Cultural Proficiency in Cross-cultural Counseling with Chemically Dependent Clients; A Strength-based Approach to Engaging Clients from Diverse Backgrounds in Addictions Treatment; An Individualized Person-centered Approach to Addictions Treatment; Utilizing Feedback from Clients to Direct Services; Culturally Nuanced Multiple Pathways to Long-term Recovery; and How to Engage Clients in Addictions Treatment Cross-culturally.

Objectives

By the end of this workshop, participants will:
1.Be aware of how to develop cultural proficiency in addictions treatment
2 Be aware of how to utilize client feedback to individualize addictions treatment
3.Be aware of 10 culturally nuanced multiple pathways to long-term recovery.

About the Presenter

Mark Sanders, LCSW, CADC, is a member of the faculty of the Addictions Studies Program at Governors State University. He is an international speaker in the addictions field whose presentations have reached thousands throughout the United States, Europe, Canada, and the Caribbean Islands. He is co-author of Recovery Management and author of Relationship Detox: How To Have Healthy Relationships in Recovery. He has had two stories published in the New York Times best-selling book series, Chicken Soup for the Soul. Mark is also an expert in helping organizations develop culturally competent systems of care.

Individuals with chronic psychiatric conditions have unique health needs that are frequently unmet in today’s healthcare system. As a result these individuals have very high psychiatric readmission rates as well as elevated rates of medical admissions. Individuals with chronic psychiatric conditions have much higher rates of medical comorbidities than individuals without psychiatric conditions. Despite this prevalence, disparities in the quality of medical care received by individuals with psychiatric conditions exist, with those with chronic psychiatric also experiencing considerably worse medical outcomes and shorter life expectancies. Managing chronic medical conditions typically involves lasting behavioral changes yet behavioral health specialists are often not involved in helping individuals to make changes to improve health outcomes. Considerable opportunities exist for behavioral health specialist to better meet both the psychiatric and physical health needs of individuals with chronic psychiatric conditions.

The objectives of this learning session are to:

· Provide strategies for enhancing transitions of care and reducing psychiatric readmission risk and emergency room utilization

· Identify common medical conditions with elevated prevalence among individuals with psychiatric conditions

· Identify common gaps in medical care experienced by psychiatric populations

· Provide strategies for incorporating behavioral health recommendations into care to improve medical as well as psychiatric outcomes among vulnerable populations
