Dear Community Leader:

In an effort to understand the potential crisis needs of special populations residing within our communities, the Indiana Office of Emergency Preparedness and Response is asking you to complete the survey which you can access using the link below.  The survey is very brief and will take approximately five or fewer minutes to complete.

Understanding the needs of all citizens and their geographical concentrations within the state will allow for more targeted responder training and resource allocation to support positive response when needed. Your participation is key to this effort.

We have attempted to access community leaders and stakeholders from all around the state in order to obtain accurate information from this survey instrument.  If you have co-workers or other community contacts particularly concerned with the welfare of Hoosiers in times of disaster or other community crisis events, feel free to forward this email to them and encourage their response to the survey.

Please know that the information generated from your response will not be shared by responder name.  It is being gathered to collectively identify, prepare for, and address the needs of all Hoosiers. Thank you for your effort.
